Please enter only once. Multiple entries will be discarded. Sweepstakes winner must reside within MIDFLORIDA's service area. Tickets will be distributed electronically so it's important to provide a valid email address. Also see Appendix A below.

MIDFLORIDA Credit Union will contact you via phone call and email (if accurately provided on the entry form) the next business day after the entry deadline if you are chosen as a winner. You must respond within 24 hours of being contacted or an alternate winner will be drawn. By entering this sweepstakes you consent to be contacted at the email address and phone number provided.

MIDFLORIDA Credit Union is giving away concert tickets for select shows at the MIDFLORIDA Credit Union Amphitheatre at the Florida State Fair Grounds in Tampa, FL. 10 winners will be selected for each show listed in Appendix A. Each winner will receive 4 Party Deck passes to the select show. The total current value of the promotion is approximately $7,870.40.

• To be eligible to win, you must submit a valid entry on midflorida.com/ampcontest during the promotional period of 12:00 a.m. April 21, 2025 – 11:59 p.m. August 21, 2025. Entry tickets may also be requested by mail by sending a self-addressed, stamped envelope to MIDFLORIDA Credit Union, Attn: Marketing Department, 129 S. Kentucky Ave., Suite 602, Lakeland, FL  33801. For each request received with valid self-addressed, stamped envelope, one entry ticket will be mailed to you for completion. Entry tickets must be mailed back to the Marketing Department and received during the promotional period to be entered into the drawing. Photocopied entry tickets will not be accepted.
• This promotion is open only to Florida residents who live within MIDFLORIDA Credit Union’s service area (https://www.midflorida.com/about-us/service-area/). This promotion is void where prohibited by law.
• To be eligible to win, you must be at least 18 years of age and have a valid Social Security number at the time the prize is awarded. Officers and employees of banks, credit unions and other financial institutions are not eligible. MIDFLORIDA Credit Union employees, officials, vendors and their immediate families are not eligible to win. The publicity in connection with the sweepstakes and the awarding of the prize are valuable to the Sponsor; therefore, persons who have been convicted of a felony, arrested for or charged with a felony, are the subject of a current felony investigation, or who have a reputation for immorality or dishonesty which would, in the opinion of the Sponsor, adversely affect the reputation or public image of the Sponsor, are not eligible to win. By entering, you consent to criminal, credit, and background checks and consumer reports to determine eligibility.
• One entry per person per email address per Entry Period. Duplicate entries will be discarded.
• MIDFLORIDA Credit Union is the sponsor (“Sponsor”). Its address is 129 South Kentucky Avenue, Lakeland, FL 33801.
• Winner will be selected at random from entries received. MIDFLORIDA Credit Union reserves the right to determine the eligibility of the winner and the identity of the winner in its sole discretion in accordance with these rules.
• You may only win once per calendar year.

• Non-winning entries received will not carry forward to subsequent entry period drawings.
• Odds of winning are based upon the number of entries. Based upon MIDFLORIDA Credit Union’s estimate of 120 entries during each entry period, odds of winning a prize are 1 in 12 for each entry period.
• You need not be present to win. 
• No entry fee, payment or proof of purchase is required to enter. Each entry will have an equal chance of winning. Making a purchase will not improve an individual entry's chances of winning.  
• If you are selected as a winner, you agree that your name and likeness, including photographs, will be listed in MIDFLORIDA’s marketing materials, including newspaper ads and social media, without compensation. 
• Prizes have no resale value. The Sponsor reserves the right to substitute a prize of equal or greater value should the featured prizes become unavailable for any reason. Prizes cannot be substituted, redeemed for cash, sold, assigned, or transferred by winners.  Prizes are “AS IS” and “WHERE IS”; the Sponsor is not responsible for the condition of any prize, and makes no warranties or representations whatsoever concerning the prizes or prize values.
• Winnings will be reported to the IRS and winners will be responsible for all applicable taxes, transfer and title charges above the awarded amount. Winner must complete a W-9 for reporting to the IRS.
• The Sponsor will send notification to the winner using contact information submitted via entry form. Winner must claim the prize within 24 hours by contacting the Sponsor’s Marketing department at 863-616-2136. If the prize is unclaimed within 24 hours after notification is sent, the prize will be forfeited and a new winner will be drawn. 
• By participating in this promotion, winner consents, in accordance with the Telephone Consumer Protection Act (47 U.S.C. § 227), to MIDFLORIDA Credit Union contacting the telephone numbers and email addresses currently on file with or submitted to the Sponsor, using natural or artificial voice calls, automatically dialed calls, prerecorded messages, text messages and any other electronic media, to communicate, deliver information and marketing messages. This consent is not required as a condition of purchasing any property, goods or services; this consent may be revoked.
• All decisions of Sponsor in regard to this promotion are final and binding. By entering, participants release and hold harmless the Sponsor from all liability in connection with the promotion or any prize won. 
• By entering, participants release and hold harmless Sponsor, Live Nation Worldwide, Inc. (“Live Nation”),  promotional agencies, franchisees, and each of their respective parent companies, affiliates, subsidiaries, successors, assigns, agents, representatives, officers, directors, shareholders, and employees (collectively “Released Parties”), from and against any and all liability for any claims, costs, injuries, losses or damages of any kind arising in connection with the Promotion, including without limitation (a) the unauthorized access to personally identifiable or sensitive information; (b) acceptance, possession, use, misuse, or nonuse of the prize that may be awarded; (c) any incorrect or inaccurate information, whether caused by entrants, printing errors or by any of the equipment or programming associated with or utilized in the Promotion; (d) technical and/or communications malfunctions, errors or failures of any kind; (e) unauthorized human intervention in any part of the Promotion; (f) technical or human error which may occur in the administration of the Promotion or the processing of entries; (g) any injury or damage to persons or property which may be caused, directly or indirectly, in whole or in part, from your participation in the Promotion or receipt or use or misuse of any prize; (h) if Administrator must cancel or terminate the Promotion for any reason; or (i) if any prize cannot be awarded due to circumstances beyond Released Parties’ control. You agree that in any cause of action, the Released Parties’ liability shall be limited to the cost of entering and participating in the Promotion and in no event shall the Released Parties be liable for attorneys’ fees or other legal costs.
• You acknowledge that the Released Parties have neither made nor are in any manner responsible or liable for any warranty, representation or guarantee, expressed or implied, in fact or in law, relative to a prize including, without limitation, the prize quality or availability.
• In the event that a concert is cancelled and then rescheduled, every effort will be made to reschedule the party deck reservation. Please note: Concert changes or cancellations are beyond the control of MIDFLORIDA Credit Union.
• Winner is responsible for the conduct of winner’s guests. Selection of concert seat location is solely within Live Nation’s discretion. Parking, travel and accommodations are not included and are the sole responsibility of winner and guest.  Ticket delivery method is at the sole discretion of Live Nation which will be sent to winner by mail, email, mobile ticketing or placed at venue Will Call the day of concert under the winner’s name. Winner must pick up tickets and a valid photo ID will be required if tickets are placed at Will Call. Winner and guest must be at least eighteen (18) years of age and must abide by all venue and event policies, to include but not limited to, showing a valid photo ID at venue for age verification and/or identification purposes if requested. Live Nation reserves the right to revoke full or partial prize from any winner or winner’s guest who it and/or venue personnel deem may, in their sole discretion, be intoxicated, be a safety risk, have violated any venue policy or law, and/or may bring Live Nation, its affiliates, performers or Promotion Entities into disrepute.  Winner understands that there are inherent risks involved in being in public spaces and that these risks may be present before, during and after the Event. By claiming tickets, you agree to Live Nation’s Terms of Use and to abide by the health and safety measures in effect at the time of your concert. 
• The Sponsor’s “Terms and Conditions of Your Account” apply to this promotion and are incorporated herein by reference, including the section “Arbitration and Waiver of Jury Trial”.  
• The Sponsor is not liable for any incorrect or inaccurate transcription of entry information or for any error or malfunction of any kind in connection with the promotion. If the promotion is not capable of running as planned for any reason, including tampering, unauthorized intervention, fraud, technical failures, force majeure, cancellation or rescheduling of event, frustration of purpose or any other cause, the Sponsor reserves the right, at its sole discretion and without advance notice, to terminate or suspend the promotion, in whole or in part, or modify it in any way, without any further obligation to eligible persons or entrants.
